What if I do not see my insurance plan listed?

It is your responsibility to determine if your insurance is contracted with Dr. Worden, Dr. Meigs or Dr. Kessler.

If insurance is not contracted then visit will be covered as an out of network provider.  You will be asked to pay for visit at time of service. You can submit the super-bill to your insurance and they may or may not reimburse you based on your plan. 

We are a micro-practice which means that in order to take limited payments from insurance, spend adequate time in a visit, we do not have staff to check insurance coverage. This is your responsibility.  If you are unsure, please have a family or friend help you.  You can contact your insurance and give them the information below for the providers at Bloom Functional Medicine.
