It is your responsibility to determine if your insurance is contracted with Bloom Functional Medicine, Dr. Worden, Dr. Meigs or Dr. Bowman. If insurance is not contracted then visit will be covered as an out of network provider. You will be asked to pay for visit at time of service. You can submit the super-bill to your insurance and they may or may not reimburse you based on your plan.
Bloom Functional Medicine is a micro-practice which means that in order to take limited payments from insurance, spend adequate time in a visit, we do not have staff to check insurance coverage. This is your responsibility. If you are unsure, please have a family or friend help you.
